My wife and I were married in Indonesia in June, and I'm starting the filing process for her to immigrate.

 

I am so afraid of messing up the forms.


My wife has not yet changed her name (didn't realize I have to be present to sign for that) but will in the future. Should I put that name (already changed on Facebook, so might be considered an alias or nickname) under "Other Names Used"?


My ex-wife is remarried. Do I put her name as her maiden name (before and after our marriage), or her new name?

Use her current legal name ( unmarried) and put hr married name if she uses it eg Facebook under other 

For you ex wife, use her current legal name. 

Read and answer each question literally ..unless there is an “unless” clause given as part of the question

Well, sort of.  In my experience, it's ok to state a married woman's name as she intends to be known.  Formalities of name change can be done later, but need to be done and dealt with on her passport by the time the case gets to NVC.

 

Former spouses should be listed with the name they used the day before you married them.  Usually, that is their maiden name but not always.

 

The day before Bill and Mary Smith married, they were Bill Smith and Mary Jones.  Bill Smith did not "marry" Mary Smith.  He married Mary Jones. who chose to become Mary Smith, literally. 😉
