Sorry to repeat these questions if someone else already asked.

We are getting ready to send in my AOS I-485, ED I-765 and AP I-131 and I want to find out three things

1. EAD I-765 since we are submitting all three forms together, in question 27.  Eligibility Category please do we choose (c)(9) since we are filling both I-485 and  I-765 together or we still have to choose (a)(6)

2. I know all forms required biometric fee $85 but are we required to include biometric fee $85 for each application, making it $255 or just one biometric $85 will cover all three applications. Because when I used  USCIS Fee Calculator said it should be $170

3. I-693 form. I had medical done first week of May and according to I-693 instruction I'm not required to have another done until after one year but I must show proof but the packet the embassy gave was collected at POE, the only record that I have is a card that shows the date of the medical, is that enough as prove?

Thank you.

The biometric fee is included in the $1225. Use the fee calculator to double check: https://www.uscis.gov/feecalculator

Even if you don't file for EAD or AP at the same time, there's still not a fee if you apply based on a pending I-485. You just need to include the NOA1 from your I-485.
