My fiancé took his medical exam, and they called him saying when he picks up his medical he will be charged additional money for a medicine but wouldn’t tell him what for.. as far as we are concerned he is healthy.. should I be worried?

He probably needed another vaccine or immunization that he didn't have. Ask him if he received a shot, that is more than likely what it is for.

He received a couple of shots while there. Maybe they missed one. They were specific and said they would give him more info when he came

I wouldn't be too worried, usually the shots or vaccinations are not included in the medical. In some countries they are, but seems like someone just forgot to collect the money for the extras.

Maybe they forgot to charge him for a last minute vaccination.  Maybe the clerk billed him incorrectly. Maybe you're worring over nothing. I choose No. 2 and 3. Relax. You're putting puzzles in your head where they are not warranted on what little information you have.
