As for her medical, she swears they only gave her one sealed envelope that she turned over to the US Government during her interview.  

No idea how we could get another copy unless I were to fly to Manila and go to St. Lukes and beg.  I hope the Government has their copy.

FYI - You can get them in less than a week by having them mailed. I've never been able to download them due to their system that is supposed to verify your identity. They ask questions that I have no idea how to answer.

 

Also, some people have been successful in getting the IRS to fax them their transcripts.

In my home country you just call or email your Panel Physician and remind them you didnt get your personsl copy of the DS3025 and they email you a copy.

I print our tax return transcripts straight from IRS website. So no need to wait.

 

If you're the foreigner/beneficiary it might be a bit harder to get an account where you can print the tax return transcripts, it can sometimes be tricky with the verification. IRS will then send you a letter with a code instead of sending it via text, which will take a few days. Even if you don't  have enough time to wait for that now, it's worth having for the future. So easy to just log in and print the transcripts.

How do you know who your panel physician is?  (No paperwork)  Will the hospital just know?  
(This is for my wife, she cannot leave the country without this paperwork being processed)

I'm the US Citizen.  My wife is the foreign national.  I'm waiting on the letter now.   The IRS says 10 days or so.

Select the request online.   Takes 10 minutes and you will have it.

 

Medical results from slec was included inside the package handed over at the POE.  All you need is to include the ds3025 that she should have received at day 2 of the exam

She never got the ds3025.  We need another copy.  The only thing they gave her on the 2nd day was a sealed envelope she was told not to open.  She gave this to the embassy at the interview never to be seen again.

Same thing happened to a friend 2yrs ago. She just emailed St. Luke's and asked for a copy of the DS-3025 or any paperwork that says she went through a medical exam. They were very quick to respond to her.

 

I would assume that all you need to provide are her full name, date of birth, visa category she applied for, and the date when she had her medical examination so they can easily look for her data in their system. 

 

As for the IRS, try looking into your email for anything that IRS emailed you about when you had the account set up with them. That's what I did to my husband's IRS account so I can access the Tax Returns for 2017 and 2018 needed to file ROC. But his was a forgotten password. Did you change #s? You could have provided an old one and you just couldn't remember.
