  • RE: I-131- Be sure to read and heed the instructions on page 2 of the form, toward the bottom of the page, right above the bar code. It simplifies filling out the form immensely.
  • RE: I-765 - The correct code for question 16 on page 1 is:  (c) (9)
  • The example forms are full of great terms to use when answering particular questions.
  • The I-864, no great hints, just have to muscle through it. Use tax return transcripts, they are free from IRS. With transcripts you don't need to send in any additional tax forms, schedules, etc.
  • Finally, to tips on how to put all of that together: https://www.uscis.gov/forms-filing-tips . We successfully used the two-hole top of paper punch method with metal prong fasteners.

The problem was this was in the sealed package she brought to CBP.  They kept it.  We don't have a copy.

The Panel Physician didn't give her an separate envelope with her XRay CD and DS3025 in it? That is your personal copy and a photocopy of that DS3025 should be submitted as back up with I485. 

Ask her to think back carefully.

They should have given her a copy of this since she needs it for her records.Tell her to sift through the papers given back to her, as @K1visaHopeful said, there's a customer copy she's supposed to receive along with her Xray CD.

I remember the CD.  Don't remember an envelope.  I'll ask her.  

If we find it, it gets copied and sent with the AoS packet?

Yes, worst comes to worst you can send it without it and they should have a copy of it in the sealed envelope as well, but sometimes they do issue RFEs for it. It's a toss up.

Yeah, it depends which employee you get at the other end.  

But one way or another, I have to get this filed ASAP.  I don't want her to be out of status, and she really wants to go home and visit.  (Homesick beyond words ...)

I apologize in advance for being pedantic: The I-131 is outdated regarding DACA. They can no longer receive DACA-based Advance Parole; this changed on 09/05/2017.

If you dont find it you should contact your Panel Physician and ask them to send you a copy so you can photocopy that and submit it with I485.

If you're missing past tax returns, you should get an official transcript from the IRS. This shows exactly what was submitted and carries even more weight. Of course, this won't work for the return you still have to file, but for past returns it's a good option.

Order transcripts for the two missing years. They are preferred over tax returns anyway.
