K1 medical summary query

Hi all -- I've been trying to find some more info about my medical summary just to make sure I have what's needed for my medical. 

 

The summary my doctor has written for me is very short -- it's about two sentences, and it just says that I am in good physical and mental health, I'm not on any prescribed medication apart from the contraceptive pill and am not undergoing any medical investigation.

 

Is this all my summary needs to say? It seems very short!

 

The other issue I'm not sure about -- they have given me a photocopy of a signed original. I presume they have kept the original for my records -- will I need the original letter with a wet signature?

Do you mean the Patient Summary Care record? They should just be able to print it at the reception desk for you, I didn't have to see the doctor. Should have subheadings on it (can't remember what they are called now), things like previous conditions, current medications, etc. Your personal details at the top.

I'm not sure -- I have all my full medical record (about 20 pages), including something entitled 'Local Record' that lists any active problems/medication I am currently on etc. However, it also lists my medical notes from all of my appointments with this GP and is seven pages long -- so I had my GP write a short summary of my current medical status. This is the document that has my doctor's signature on it, but I don't have the original -- just a photocopy.

 

In medical confirmation email, they also ask for:

 

Summary of medical history from GP (NOT full medical records) and reports covering any medical conditions, disabilities, and prior serious illnesses you have been treated for or anything ongoing.

Sounds like the local record is the right one. Don't worry about the note with the doctor's signature, don't think they are going to want to see that. For my medical I provided I 3-page summary, which sounds a lot like your seven page one.
