My fiance keeps asking me about the medical interview and what will happen when they see scars on her wrist. She has previously harmed herself in January, about 3 weeks after I sent the packet. I received a notice from USCIS on 23 May 2019 saying my case has been approved and will be forwarded to the NVC for processing. I started preparing her and myself for the coming steps. 

I told her about the medical examination and what it will most likely entail based on other member's posts. She is understandably concerned regarding the possibility of being denied based on self harm. She has two small scars on her wrist, one is very light from a shallow cut and the other a good bit darker. She has never done this before and has never done this since. 

My question to you good people is this: what do we do about it? She has seen a psychiatrist and been prescribed lithium for anxiety. She was not diagnosed for depression or suicidal thoughts. After she realized what she had done she checked herself into the hospital, stayed overnight for evaluation, and was released the following morning. The incident occurred in a different country, and as such the incident is not recorded in her medical records in her home country. There's no way to prove when the incident occurred. Does the amount of time passed since the incident have any bearing on the doctor's or embassy's decision to grant her the visa? What is the best way to approach this issue to set her up for success?

All she can do is tell the truth.  Potential harm to self or others is actually a reason for inadmissibility.  Best wishes and I hope she gets whatever help she needs.  Incidentally, an experienced clinician can in fact estimate the age of scars.
