Currently working on the form, few questions I have..

 

My family name (last name) is that before I got married? My name is different now-I took my husbands last name

and 

When it comes to mailing address it asks for "in care of name" what does that even mean??

Only write the “in care of” name if you have one. The fact that you don’t know what it is suggests you don’t use one. So leave that blank.

My ceac account says I paid for I864 but im filing I864EZ??? 

 

I PREVIOUSLY HAD AN ATTORNEY BUT WE TERMINATED HER.

Not an issue. In this scenario "I-864" is an umbrella term.
