Hi, my fiance and I received our NOA2 nearly 4 months ago and it is going to expire on the 21st. From what I've read it is easy to get an extension in four month intervals. Unfortunately we are having issues getting his medical information and him and I are both having separate medical issues of our own on either side that have sidetracked us a bit (me - US and him - London). We were never able to schedule a medical exam for him or the interview, however, we are confident that we can do this within another four months. What are our steps to asking for an extension? I've read that a simple email would work, but is that correct? And I believe that it would be from me to the UK, but where could I find the email for that? Also, how much information is necessary for me to give for why we haven't been able to proceed, and what are the chances of us being denied the extension? Thank you.

I just answered the same question an hour ago, here is the info from usembassy.com below.

Let’s say you finally find a slot open for the K-1 visa interview, but it’s past the 4 months that the I-129F is valid. That’s fine, just be sure to send a quick email to notify the embassy to extend your validity. In all likelihood, once they see that you’re actively scheduling an interview, they will automatically extend your case.

Here on a K1? Need married and a Certificate in hand within a few hours? I'm here to help. Come to Vegas and I'll marry you Vegas style!!   Visa Journey members are always FREE for my services. I know the costs involved in this whole game of immigration, and if I can save you some money I will!

What are our steps to asking for an extension?

You can either email or make a phone call to the Embassy.

I've read that a simple email would work, but is that correct?

Yes

And I believe that it would be from me to the UK, but where could I find the email for that?

The contact can come from either the petitioner or beneficiary.  Use their website for contact information.

Also, how much information is necessary for me to give for why we haven't been able to proceed,

No. You only need to provide a short statement as to what is preventing you to complete your interview. It would be advisable if you where both able to include in your email two letters, one from each of you, that states that basically you both still hold the desire and that you intend to marry and are free to do so.

and what are the chances of us being denied the extension?

Less then .0001%.
