Right. Except for all the cases where people have done absolutely that and completed the green card process within the US. We see it on VJ quite regularly where somebody changes their mind after entry. I'm certain CBP sees people trying it much more often than VJ does.

Only men trying to illegally work or to overstay must be a new thing. My wife's female friend who tried to do exactly this and was denied should try again due to this new development.

They got visas to visit after their circumstances convinced the CO that they would return home. The reason for the visit doesn't matter - medical, in-law vs immediate relative, etc.
