If you don't remember the exact day and month that you applied for the tourist visa before, just put your best estimate of the date, and add a note in the additional information section at the end to explain that you can't remember the specific date.  Hopefully your memory is good enough that you will be close.  They will have all that information on file anyway in their system.  Good luck!
