The best is to use a scanner with PDF format.  For some reason, you cannot have it scanned, you can simply take picture then use Microsoft Word > Save As PDF. 

 

Side notes,

1. Only include the biographic page (the page with the picture)

2. Make sure the text are align horizontally so reader can read without rotating the passport.

There are many apps that can complete such a task. You can use google to search. Go to the store and look for apps, I like to look for the highest rated apps to try as they seem to work the best. Also, you can do it on the web here https://www.passportphotoapp.com/
