So here is my example:

Timeline of Beneficiary name changes:

03/xx/197x

Irina Konxxxxx Eniaxxxx - Maiden Name

04/xx/199x

Irina Konxxxxx Devxxxxx -  Married to Aleksey Vikxxxxx Devxxxxx in Tarxxxxx, Russia

12/xx/199x

Irina Konxxxxx Devxxxxx (no name change) -  Divorced from Aleksey Vikxxxxx Devxxxxx in Tarxxxxx, Russia

(Note: This 199x divorce document mistakenly misspells Irina’s surname as “Deixxxxxx” upon dissolution of marriage)

10/xx/200x

Irina Konxxxxxx Gavxxxxxx - Married to Viktor Vasxxxxx Gavxxxxx in Tarxxxxx, Russia

07/xx/201x

Irina Konxxxxxx Devxxxxx -  Divorced from Viktor Vasxxxxx Gavxxxxx in Tarxxxxx, Russia

01/xx/201x

Irina Konxxxxx Murxxxxx -  Married to Randall Josxxxxx Murxxxxx in Cedar Rapids, Iowa, USA

We used the American style of MM/DD/YEAR

Very good question.  I would go with a "can't go wrong answer" which is to use the style found on a US Visa:

DDMONYEAR

Examples:

03JAN1996

12FEB1998

08MAR2000

24APR2002

31MAY2004

18JUN2006

27JUL2008

01AUG2010

21SEP2012

11OCT2014

30NOV2016

08DEC2018

For your other question, we did my timeline as part of our several different USCIS petitions' cover letter.

Do not overthink, it should be a simple letter with main focus on timeline.  Add the case #,  beneficiary's name, and add a title to clarify this timeline is for who.  Keep it simple.

I do not think there is any format you need to follow, it is something that you can create one yourself.

Yes. Be simple and concise.  I do not see explanations are required as they just want to verify if a new marriage had happened before a divorce was legally finalized or (less probability) how long between a divorce and a marriage.
