There are links for appointment registration for each location, that appears to be the only online information you can complete.  Any other forms will be completed at the CFO office you choose.

Not sure that you can reprint the barcode later.   If you can see it on the screen use "print screen" then paste that to a word processor  and then you can print that

This is the same error that I was getting, and your solution fixed my problem as well. I needed to use "my" usa address even though i haven't lived there in many years.
