CFO SEMINAR. Does have an schedules appointment but can’t find the registration form link

Hi guys! How can I find the link of registration form for cfo so I can fill it up? I already have an scheduled appointment made but i haven’t seen the link. Can someone help me. Thank you! Please see photo for reference

Thank you mr. Hank for your reply! Should I download and print the form before filling up? Where will I send it? And after that where can I find the “barcode confirmation” which is needed to be print

It is an online form, complete and click the submit button.  Once you complete the form you can then print out the confirmation page with barcode.

Is that the one I fill up after I choose an appointment date with my photo in it? I haven’t received any barcode confirmation on my email 😓

Yes.   If you didn't get the barcode confirmation you are missing some information with the form.

Can you still access the online form you were completing?  If not then I would say to start over.

For me I having error codes, don't know what could be missing. 

Error message:

String or binary data would be truncated.
Please check the Data Entered by the Client.

Found the problem after several days and many unanswered calls. I had to use my USA address (parents) for the petitioner part instead of my Filipino address since she is going to the USA. Strange because I lived in the Philippines for 2 years.
