It is an online form, complete and click the submit button.  Once you complete the form you can then print out the confirmation page with barcode.

Is that the one I fill up after I choose an appointment date with my photo in it? I haven’t received any barcode confirmation on my email 😓

Yes.   If you didn't get the barcode confirmation you are missing some information with the form.

For me I having error codes, don't know what could be missing. 

Error message:


String or binary data would be truncated.
Please check the Data Entered by the Client.

Found the problem after several days and many unanswered calls. I had to use my USA address (parents) for the petitioner part instead of my Filipino address since she is going to the USA. Strange because I lived in the Philippines for 2 years.
