Do you request a new social security card when you get your green card?

So what documents do one need to go with? Is the greencard and the SSC enough? And will there be a need to fill any form?

I just did this a few weeks ago.  Yes, you need your green card and current SSN card.  You can choose to fill out that normal form you would to request a new social security card. I didn't do it, and it worked out, although the SS employee was continuously passive aggressive for not filling out the form.
