Everything looks good!  I submitted my AOS for my wife and step-son in January, so I know what you're feeling about making sure everything is in order before sending.  And be sure to make a copy of the ENTIRE package before sending, for your own records.  Good luck!

Hey, I remember you advised me the same but now when I got my green card I can confirm that only one copy of marriage certificate if needed. So basically only I-485 needs a marriage certificate and all other papers while I-765 and I-131 require only bare minimum as IStorM mentioned. I really don't know why they asked you this. From my experience I sent only the papers that were mentioned in the instructions and never got an RFE and no problems at the interview either.

thank you very much for all your input. another question is what kind of organizer we should use to put all forms together?

Please be kind to the poor person that has to open and disassemble your application.

Question- do we need to send copies of ALL of the pages of the passport or just the pages that have the biographical information and the visa/entry stamp?

Follow the instructions that come with the form. It will tell you what you need. (It's just the BIO page btw). You used to have to download it separately

"DS-3025 Vaccination supplement (Vaccination Documentation Medical Worksheet)"

So basically I need to take another vaccination upon my arrival to the US and the doctor will fill that form that I will attach to my AOS?

You should have received a copy of the DS-3025 at the medical.  If it marked complete you don't need anything else.
