hi again everyone and thanks for those who replied on my previous post.

i have a problem with my wife's DS 156 form, i already sent her the form and she noticed something. here we go:

on question number 6,7,8 and 9, I or (actually my lawyer fill it up this way)

6.) Surnames (As in passport): Iverson

7.) First and Middle names (As in passport): Michelle Tan

8.) Other Surnames Used: Tan ( this is her last name prior to marriage with me)

9.) Other First and Middle Names Used: NONE (yes, we put None)

the problem is on question number 9, me and my wife are wondering if should we put her first name (Michelle) and Middle name used ( this is her middle name prior to marriage or her mom's last name which is "SANTOS")???

still... the MAIN PROBLEM that bothers us is when my lawyer filled up her I-129f and I-130 forms plus her G-325A. On the portion where it asks for "OTHER NAMES USED", our lawyer put "NONE" in there. ( he thought those applies for previous marriage names or alias).

so... IS IT OK if i leave the DS form that i sent her as it is? OR send her a new one wherein i will put MICHELLE SANTOS on question #9?

me and my wife are so bothered with this since if we put Michelle Santos on number 9 then we put "none" on her I-129f/I-130 form as other names used, the Consul might ask why did we put none on the I-129f (K3) and NOW place a name on question number 9 on the DS156.

WHAT SHOULD WE DO GUYS???? HELP US!!

Guys my wife and i need your help on this. Pls help us.

Honest errors such as yours is not a big deal to the embassy. Some people have had corrections made during their interview. Your wife should mention the error during her pre-screening with her Filipino interviewer and then again to the American consul. Just tell them that the lawyer made a mistake in putting "none" in "other names used" in the I-130, I-129F and G-325 and that you only noticed the error recently.

The most important thing is to correct an error as soon as it is found out.
