For AOS you will take the DS-3025 to a Civil Surgeon to be transcribed to the I-693, if the Civil Surgeon determines it is necessary he will administer the vaccine.  Check the DS-3025 (on the right side) to see what box is checked as to status regarding that vaccine.  If needed then maybe get it before visiting the Civils Surgeon.
