Just wanting to verify filling out the DS11 form correctly for my newborn daughter, which we are submitting to the US embassy in Thailand with the CRBA form and other materials. 

I understand my daughter is the applicant.

Does this mean that every section is to be filled out as they pertain to her?

Such as section 9 - list other names you have used or Section 11 - have you ever been married, etc... Height, Occupation, Employer, etc - all for the applicant (my baby daughter)?

 

As her parents, my wife and I put our names for section 10 - parental information but everything else is answered or filled out for the baby? 

 

this form seems goofy for filling out for a baby!

Yes

Answer "No." If marital status is asked then put "Single" or "N/A" https://www.travelandleisure.com/articles/how-to-baby-passport
