Hi all!

I have a question regarding item 37-38h. The style sheet instruction states that you should "provide the date of your beneficiary’s most recent entry in the United States, his or her I-94 Arrival-Departure Record Number..." but the form itself says to only fill this out if you are currently in the US, which I, the beneficiary, am not? What should I do!?

Thanks!!!

1.  Answer question 37

2.  Leave 38a-38h blank

Additional question - what if you didn't receive an I-94 when you entered the US?  do you just put your arrival date?

You can download an I-94 at https://i94.cbp.dhs.gov/I94/#/home
