I have a possibly dumb question. Since his first job in NY my husband worked for employers in NYC but his W2s and paystubs had another name because he was a contractor and these addresses are in GA and FL and one more somewhere else. Which address do I put under the name of the employer, the address in NY or the address that's on W2? And if I put the address that's on W2 should I include an explanation that he was in the NY office?

I had similar issue, the employer address should be the same one on W2
You can add an explanation as a document you can attach

Is office location referring to our local office or the service location where the application is being processed?
