Hi - I am scheduled for an interview April 16, 2019.

On the form I have selected a name change, i.e., my current last name becoming a middle name.

My question is can I request the IO during the interview to amend that section now that I no longer want a name change?

Thanks!

Yes, they can. In fact, I think they will double check with you anyway.

In my interview, my form indicated no name change. She asked me again to confirm or if I changed my minds. I said no.
