Complete the missing vaccination and then have it added to the DS-3025 confirming that the medical staff has marked the form complete. This will save time and money when you go to submit your AOS packet. 

Thanks so much for the help as always!  Will get him to give the office a call and get that sorted out while I start figuring AOS stuff for when he finally comes over 😊

Go to guides on the tab above and look for AOS for K-1. S/B on the right hand column of choices.

Anywhere that it asks for my wife's name should she put her new name or considering that her new last name is not recognized by any government, ID, or institution yet should she just stick to her maiden name on all AOS forms?

I am assuming it is better to with her new last name so it will be recognized after AOS, on her travel document, and with her work permit. 

I just want to make sure it is ok with USCIS for her to put her new last name.

Some forms ask for "other names" where we will add her maiden name there but the I-131 does not give the option for other names so is it ok for her to put my last name there also?

Everything that ask for name it's time to use the new name except, when the maiden name is asked.

I-693  You take this form and the DS-3025 Vaccination Record to a Civil Surgeon (from the USCIS list).  The Civil Surgeon will transcribe the DS-3025 vaccinations to the I-693, then the I-693 will be put in a sealed envelope and given to you to accompany the I-485.   A new medical examination is NOT required,  your medical from St. Luke's is valid for ONE year.

 

This is on Hanks site, but I have had several others say all you need is the DS-3025 completed to send in for AOS.... @Greenbaum

Make sure that your DS-3025 has the "K1 Visa applicant has voluntarily completed vaccination requirements". The only times I've seen they need to transcribe it is when this was not checked by the doctor.

He never had that requirement before.  Maybe something new.  As long as the DS-3025 states complete your good to go with just the DS-3025.
