DS - 160 Form

Hi everyone

 

I'm pretty scared right now

So I have already finished filling my D's 160 form,I am filling for I and my dad our date are on the same day

 

My question 

 

1)I want to complete my father's D's 160 form but I can't gain access I am told the surname and date of birth is not a match but they all correct,the security question is correct

So I don't know why I can't gain access

 

Another question since I'm travelling with my dad and I have already filled the member form on my us travel doc is there a need to fill another us travel doc form?

 

I really need help
