Where do i get I-693A?

I checked the USCIS website, I only see an I -693 form, does the civil surgeon only have this paperwork?If possible where can I download form? also what is the DS-3025, ?

A simple google search for the supplemental form to I-693 brings up this page.

I believe it is sometimes the case that the CS has all the forms in his office to fill out but you can print and take that with you if you want.

The DS3025 is the vaccination worksheet which you are supposed to get

from the panel doctor where you had your medical.

You take it to a civl surgeon here to have it transfered to the I693.

You will get it in a sealed envelope and ask for a copy for yourself.

The fees vary greatly so check around, reasonable fee is about $10-$50.

True that - you get it from the civil surgeon him/herself.

Ours came in a sealed envelope to send to immigration and a copy for us to keep.

Its always the case that the civil surgeon has the form, which is why USCIS don't have it on their website, along with all the other forms. You can't fill it out yourself, so there's little point in downloading it.

In my case i did not take the format of I 693 A with me to the CS.

He had it there..

Be sure and shop around for a civil surgeon as they all seem to charge a different price for the same thing. So don't pay more than you have to.

As StormnRolly wrote, you can get the form by Googling 'I-693 Vaccination Supplement'

You will not get the form by Googling 'I-693A' because the form is not designated I-693A - the I-693A designation is a VJ fabrication, it does not exist 'in real life'.

I also think it's better to 'shop' around for a CS. That is what we did - and it was worth it.

The first doctor wanted to charge my husband for an immigration appt - the one we went with transferred his DS-3025 onto the I-693A in a few minutes and charged us $25.
