Japan Police Clearance help for IR-5 (Parent Visa)

Hello All,

PLEASE HELP!!

I am in the process of bringing my Parent here via IR-5 Visa. 

 

Wanting some advise about the Police Clearance -

While filling out my Mother's DS-260 - "Previous Address since age 16, seems like she has lived in different cities 4-5 months at a time (Fukuoka, Kumamoto, Aomori, Tokyo). Does she need a police clearance from all these different places or just 1 Police Clearance (from Tokyo) that will cover all? Has anyone experienced going thru this?
