This is what I pulled from the USCIS website under the 'required initial evidence' section:

 

  • A signed statement from an officer of the bank or other financial institution that states when you opened an account, the total amount deposited for the past year, and the present balance of such account
  • A signed statement from your employer on business letterhead that states the date and nature of your employment, salary paid, and whether the position is temporary or permanent
  • A copy of your last income federal tax return filed, or a report of commercial rating concern (if self-employed)
  • A list containing the serial numbers and denominations of bonds and names of the owners (if applicable)

Ask your employer or the HR departement in your company, they'll write one

Don't have experience with co sponsor, but would think so yeah. I would also advice you to read the instructions on the USCIS website, a lot of your questions are answered by simply reading the instructions https://www.uscis.gov/i-134

Yes......a joint sponsor must submit an individual form and appropriate support documents.

In summary, it took 13 months for approval of the CR-1.  It took 44 months for approval of the I-751.  It took 4 months for approval of the N-400.   It took 172 days from N-400 application to Oath Ceremony.   It took 6 weeks for Passport, then 7 additional weeks for return of wife's Naturalization Certificate..

The co-sponsor has to complete a separate I-134, so all supporting documents are unique to that I-134.  Therefore, the co-sponsor must have separate supporting documents.
