NVC switching First and Middle Name; Congressional Inquiry

The NVC has a history of switching the First and Middle name on cases where the country passport does not follow the standard of LastName, FirstName MiddleName.

 

I am working with two U.S. Congress Senators on this issue and they are bringing this to the attention of the Department of State and NVC. The big question is how often is this being done at NVC and what countries are affected.

 

If the Administrators here will give permission, I want to gather information to present to Congress.   

 

I am wanting to gather information on cases from January 2018. First I just want to know the month and year the case was created at NVC, the country and how the name was changed.  This is a serious issue and can cause several problems. I have worked on dozens of cases and have seen this issue to many times. I spent 22 months getting my wife's name corrected. It caused issues with Social Security, IRS, DMV and others.  I have three other cases at NVC and the names have been changed on them. 

 

If the Administrators will allow, please post a reply with non-confidential information. I am hoping to gather enough meta data to show this is a bigger issue than NVC admits.
