I - 485

1. If my Fiance keeps her maiden name after our marriage and uses it on all the AOS documents will there be any problem with this? Just wondering if USCIS has any preference on this? Also wondering if her keeping her maiden name would cause any conflicts when we go for the interview and present evidence of common bank accts. ownership, etc.

2. In the instructions of the I - 485 in regards to the vaccination records. It only states that a "vaccination supplement" be included. It does not state exactly what that is. I know there has been some discussion on the board about a Civil Surgeon transferring the info to an I-693 A form and submitting this. Can the copies of the pages of the Yellow Book / International Certificate Of Vaccination be used in lieu of the I -693 A? I was told by my Chinese Lawyer that this would be adequate.

Part 5: Sponsors Household size.

1. I am also submitting AOS package for my Fiancee's 16 year old daughter.(K-2) Should she be counted as a dependent in this section on my Wifes application, giving a number of 3 for my household size? ( Me, Wife and Daughter) ...........or excluded in this count and only listed on her (the daughters) application. This would give a count on the Mothers application of only 2 and a count on the daughters app. of only 1.

Or should I count myself again on the Daughters app. or not? Or will they just know to add the numbers together from the 2 apps. to get a total of 3 in the household?

I know this count is to determine if the household size meets the Poverty Level Guidelines......but am confused on how to make the correct selections because I am submitting documents for both Wife and Daughter.

It won't cause any issues, although if you have an interview they may ask your wife why she didn't take your name. At least, they asked my wife this!

maiden name is nt a problem and yes as replied by Dr. Lha you might be questioned on this at the interview.

I have kept my maiden name.

There are people who got RFE for not submitting I 693A. Though some have got away with submitting the DS 3025.

I got my DS -3025 vaccination record transferred to the I 693 A supplement today by a CS. I did not have to undergo a complete medical since my vaccination requirement is complete.

Its a small effort and worth the trouble so that you are safe from getting RFE'd bu USCIS..

3 is your household size, on all apps.
