Just double checking that I understand this...

Ok, so my fiance was NOT given a copy of the DS3025.  Everything was put in a sealed envelope and sent to the consulate for his interview.  He even asked if there was any document that he needed to take with him and the doctor said no, it was all in the envelope.  He was only given a shot record from the clinic showing what vaccines he was given that day (the 4 that are required).

So we need to get a copy of the DS3025 from the doctor in Brazil to send in with the adjustment of status application?

No.  If it was included in the sealed envelope you are good to go.

YMMV

Yes. Call or email them.

You'll submit a photocopy and bring your copy and a photocopy to the interview.

No.  Including a copy of the ds3025 with the i485 does nothing

I beg to differ.

You cant say it does nothing when submitting it could remind your IO at your AOS interview when they scroll through your file, that you have completed an overseas medical thus reminding them that if they dont have it with them to go look for it thus clearing up the medical issue.

It acts like a signal flare in the pile of paperwork they will be reviewing at your interview.

If they dont see it, they may not make the connection with you being a K1 and having completed the medical overseas and not even talk about the medical exam requirements with you until after, when it's too late for you to explain and they issue a checklist to complete a new medical.

The DS3025 photocopy is a great submission and has helped many to remind ignorant IOs about the K1 exemption to having an I693 or US medical.

I love how people make up "what if" scenarios without any empirical evidence.  Based on that logic we should submit everything including the kitchen sink.

I do too because it helps ppl to understand how ignorant IOs are on the K1 AOS process.

I've seen many scenarios of ppl here where had they simply submitted the DS3025 with I485 and either the IO came across it during the interview or they force fed them another copy of one then, it could have saved them a lot of stress and $$ for a I693 after the fact.

I agree with @K1visaHopeful.

Sometimes (it's becoming more common recently) that USCIS misplaces documents. If not submitting DS3025 with AoS, then atleast have it with you for the interview to avoid the threads asking where to get medical.

Besides one page isn't 300 pages. It doesn't hurt to have it sent with AoS, or bringing to the interview.

Have people lost site that the affected individual doesn't have the ds3025 and was told to get it because they need it.

If you have it you are not harmed to include it, but that is not the situation at hand

I'm just commenting in general.
But yeah, if one doesn't have a completed DS3025 - You'll need I693.

If it was complete, but you don't have a copy - call the institution that did the medical, they should be able to issue a new copy for sure.

But sure as hell, medical is needed sooner or later.

Just because the visa holder doesn't have a copy of the completed ds3025 doesn't mean it  wasn't included with the Visa medical packet.

The OP has been advised to contact their Panel Physician to get it so they have it as back up....pages ago.
