She makes an appointment like anybody else. Her legal status is not a bar for medical care.

As for paying for it...presumably you can marry and add her to your insurance plan? I'm not sure how this would work while she has no authorized stay (presumably not an issue), but certainly once she files for AOS she should be fine. Note that an SSN is not needed for insurance...you just may have to work through it with the insurer or HR on how to do so.

First thing is to check if your fiancé’s J1 visa has a two-year home country physical presence requirement.  If she is not subject to the requirement, you can apply for an adjustment of status immediately after your wedding. And if she is subject to the requirement, a waiver can be requested on certain bases. Thanks Ellis ! She is not subject to the 2 year home requirement, So we are working on filing her paperwork in the next week !
