Where exactly is this "COMPLETED" check box? I've just read the DS-3025 from top to bottom multiple times and I cannot find any "COMPLETED" check box.

 

Also, please don't yell at me for "resurrecting a zombie post". This post is less than one year old, and it is the most recent post I could find on this topic.

The name of the form ds-3025 is "Vaccination Documentation Worksheet". "Give Copy to Applicant"  it is a check box " US VACCINATION REQUIREMENTS COMPLETE" 

 

It is located at the bottom middle near the physicians name.

The DS-3025 doesn't work for anyone unless it is marked complete AND included in the "do not open" packet with your visa medical report.  Submitting a copy of your ds3025 with the i485 doesn't do a thing

The DS-3025 is included in the "do not open" packet by default, right? So it's only not going to be included due to human error.

There's no way for us to know if it wasn't included other than to receive an RFE saying so, right?
