I don't understand one thing about the vaccination documentation worksheet..

I have all the vaccinations as required for my age(30) yet at the bottom of the worksheet the doctor has ticked on vaccination summary incomplete....this is confusing!?!

Today when i called up of the doctors in my locality he agreed to give me the vaccination supplement provided I have the TD and MMR taken, which i have in my home country.

Further i checked up the vaccine requirements for my age and found that I have all of them taken...

So why is it ticked on "vaccination history incomplete" and "applicant might be eligible for blanket waiver"

confused!!

I've been meaning to ask the same thing. The same box is checked off on my vaccination document from my K-1 medical, but I have all of my vaccinations as well. :huh:

Did you have the vacinnations at the medical or before it ? if before and did not have vaccination records to be transcribed to the DS-3025, the doctor would check vaccination histry incomplete.

Well, if you have proof that you've had all of the vaccinations, take that, with your DS-3025 to the civil surgeon and have everything transcribed to the I-693a. Shoud be pretty simple.

Completed series had been checked off for the required vaccinations on the DS-3025? I'd bring it along with any vaccination records you have to be transcribed by the civil surgeon to the I-693a. if you have the records, but the civil surgeon will not transcribe them, try another one.

In my vaccination worksheet completed series has been ticked.

But Vaccination history incomplete is also ticked at the bottom....

Apart from the DS 3025, I do not have any other vaccination card or something else to prove my vaccination history..

I have all required vaccinations taken as evidenced in the DS 3025/

so...what does it mean...will the doctor be satisfied enough to prepare the I 693 A for me?

We just passed the AOS interview on May 7, 2007,and received our white cards (green) on the 19thof May, less than two weeks. My wife's vaccination worksheet also was checked not complete. The vaccination worksheet was approved for the K1 Visas and also approved for the AOS. It did also give us some concern but figured that if it mattered we would have received a letter requesting further information.

The same thing happened for my husband. His worksheet was marked as not complete, and we never had an issue with it.

I figure we did everything we were told to do and if USCIS needed more, we would find out eventually. But we never received an RFE for anything.

ONE DAY AT A TIME....

REMOVAL OF CONDITIONS

12/30/2008: Overnighted I-751 package to VSC

01/06/2009: Check cashed

01/06/2009: NOA (arrived 01/09/09)

01/23/2009: Biometrics appt letter received

01/31/2009: Biometrics scheduled

05/20/2009: APPROVED

06/23/2009: 10 Year Green Card arrived
