Are you sure your wife didn't refile?   I'm so confused...your status a while ago said they were reviewing information you sent even though you never received a NOIR.  The status updates you get are not typical for what you think is going on in your case.   Reviewing information is typical after they received something, RFE response or rebuttal.

When my case was reaffirmed the status stated "case forwarded to the department of state".  I never had a statement of approved (even the first time).

It is very possible it was reaffirmed without them needing anything at all from you.  It does happen... but you definitely need to call them to find for sure.

Yes sister my wife didn't refile and before they said correspondence was received and uscis is reviewing it and we will notify you by mail when we need anything from you.so we ask to our senator and when our senator make inqury on behalf of us.she got that response Your constituent’s Form I-130 completed extended security screening and background checks, and the Form I-130 was subsequently assigned to an adjudicating officer and placed under supervisory adjudications review. Your inquiry was forwarded to the adjudicating officer and supervisor for their visibility. Your constituent should receive correspondence or notice of action in the next 15 business days, if not sooner. If they do not receive correspondence or notice of action by that time, please inquire with our office for a follow-up inquiry. and After that email my case status updated on uscis website case was approved.we are calling the uscis on Monday.if they don't give us NOIR that's mean our case was approved not reaffirmed maybe they only use that who recieve NOIR that's what I am thinking but maybe I am wrong about it.
