Wendy and I will be giving another shot at the DS156 form online today. Looks like the online form is now working again.

1 more question on this form.

I know that she will need TWO DS-156 forms filled out. The DS-156 form has a unique barcode and barcode number for each form. Will she need to make copys of one of the forms. Or does she need to have TWO seperate DS-156 forms??

For example

TWO DS-156 forms with barcode XYZ5

OR

ONE DS-156 form with barcode XYZ5

&

ONE DS-156 form with barcode OMN5

Thank you

Hello, not too sure if this will help, but there are two different 156 forms.

one is the 156, and one is the 156k.......both will be needed.

Good luck.

David

wow, we have already recieved our Biometrics appointments. It is only 2 1/2 weeks away.

It seems to be moving rather quickly.

This is great.

David,

Thanks. I am aware of the 2 forms. However the embassy in Colombia needs two of the DS-156 form. I'm not sure if we should just print the form out twice. Or get 2 seperate DS156 forms that each have their own bar code?

You should doublecheck with the consulate or someone who has been through it, but my understanding (and how I did it) was that you are supposed to print out the form twice with the same barcode (or make a copy of it). You don't want two separate barcodes because the barcode is tied to your information and having two would complicate it.

Lucho:

I responded to your question on the Colombia Club thread. Last time I checked, Bogota was still not using the barcode system, so the answer to the question is irrelevant. However, if you want piece of mind, print two DS-156s with the same barcode.....because they are supposed to be identical (for those embassies that use it, the barcode provides information about the beneficiary....thus both should be the same).

You should be able to print out two copies of the same barcode when you're filling the form out. :) Good luck!
