I am filling out FORM SD-156 online and for some reason it wont go through and says "Internal server error" Is it mandatory that we need to fill out this SD-156 form online? and generate the barcode from there?

Another question, What form should I bring when I go to the bank to pay for Applicaiton fee?

I see that one is not working

Make sure you have the latest version of adobe reader installed on your computer, should be 7 or 8

I want to repeat the OP's question about which form acts as a receipt for the application fee - for we have the same question. We have seen two different answers from different sources.

I explicitly remember reading that the bank annotates one of the DS forms upon receiving the application payment. Unfortunately when I read this, I was recovering from surgery and was on some pretty ###-kicking pain meds - thus the memory deficiency.

If you search your consulate website, I'm sure you'll find the information there explaining exactly what form needs to be provided . . .

PROOF OF PAYMENT. The application fee for a K1 or K2 nonimmigrant visa is US$100 (or the

Philippine peso equivalent at the prevailing exchange rate) which should be paid by each applicant at

designated branches of the Bank of the Philippine Islands (BPI) and Citibank (see attached list). The

bank will validate your payment on the DS-156 Nonimmigrant Visa Application Form which you will

find in this packet.

DS-156 needs to be filled out online to generate a barcode. As for the form that the bank validates upon payment, it is the DS-157, not the DS-156...

Wow!! I didn't even know that i got a lot of response from you guys. I didn't check this topic i posted until today so it's like almost after a year.
