Yes if you filled out the application and answered all the questions yourself on there then yes you have to sign it and say you prepared it, but if you printed it only then no. I wish you all the best on your journey!!!

I did one for my SO, the part about prepared by someone else has to do wait a lawyer, I am not a lawyer so just say NO, anyway my fiance just did another one by herself.

A few other odd things about Guangzhou, the people at the medical center sometimes insist on the hand written DS-156, and they like to toss the third page containing the data barcode. They have been told to not do this by the consulate, but the medical college hospital keeps doing this.
