I had to read it because I was looking for requirements for my Bio dad, as I was born out of wedlock. (They were the OG 16 and pregnant). Since I have his last name , I am considered legitimize, that is another reading for me.

My my other issue with bio dad aside from history of drug abuse was his name all his life was different from what his birth certificate says. Again more reading.... :(

Getting the name fixed is possible. My wife's mother's first name was wrong on her birth certificate as well so she had to get it corrected so she could get a passport in the right name.

I don't know the process to do so myself, though. I'd have to ask her. If you can't figure it out, let me know and I'll ask her. Hank is also the unofficial PH guru. ;)

I was there last month and we went to the local civil offfice to finally get through the process, I was ready, butttttt I almost lost my when we were told my dad has to file it in cebu (his birth place) because they have no capacity for national publication. Efff me!!!! I was quoting their website that says it is impractical to go back to the birthplace but I was told of the same thing. This is in Angeles city. So now I’m sending my dad to cebu. Coz I’m rich—not!
