Scroll down and you'll see Claims. 

 

Choose the form Application for Provident Benefits (APB) Claim (HQP-PFF-285V03).

Or choose Provident Claims on the left under Citizen's Charter.

Finally found it! Thank you so much😊 One more thing, can I just submit either a copy of passport with the visa or a notarized sworn letter? Or should be both?
