Hi,

I have a couple of questions about the I-134. The form asks for the total market value of any stocks and bonds that I have. Would it be OK to include the amount for mutual funds as well?

Also, under life insurance, what does "With a cash surrender value of" mean? Is that the value of my life insurance policy? If so, wouldn't the figure for that and the previous question ("I have life insurance in the sum of") be the same?

1st question yes.

2nd question: Some life insurance policies have a "cash out" value if you needed to borrow against them, or surrender the policy for emergency money. That's what you would put there if its available to you.

Thanks everyone for your responses! I really appreciate the help. I didn't realize that I don't have to provide this information if my income meets the guidelines (which it does).

So if I don't want to provide the figures for the stocks and bonds and the life insurance, what should I enter in the blanks? Would 'N/A' be sufficient?

I left all the other boxes blank. But N/A would be Ok also. Good Luck
