Hello E'body..

This is my third post/query in the last one hour..been trying to research and process my AOS papers.

This is a query regarding the medical which i need to submit along with my AOS papers.

As per the guidelines in this site, it talks about some DS-3025 (medical documenting vaccinations) which the doctor in my home country where i did my medical last May/June was supposed to hand over. All i got from the hospital is a paper documenting the requirements for routine vaccination of immigrants. It does not have my name, pp number, other details mentioned, neither has the doctor signed it. I was never aware of the DS-3025 then and hence never asked for it.

Does'nt look like that paper would suffice for a DS-3025 in any way.

Moreover, the sealed envelope was obviously never returned at the Port of Entry at LAX. I guess it is never returned to anybody, not sure though..

Currently, in order to get my I 693A, i do not have any medical documented vaccination record which i could present to the doctor..

My Queries..

1.What do you think I should do about it?

2.Contact the hospital where i did my medical and ask for a DS-3025?

3.Is the DS-3025 have a format? I never got it in the USCIS site. What is documented in this form exactly?

4.Where do i get the format for I 693A, not available in the USCIS site again. The 693 is there only.

5.I got my medical done May end last year in my home country...is there a 12 month validity for presenting the 693 A with my AOS papers? If there is, what does it mean? Do i have to get a full medical done ? what are the costs? What is the process?

Request for all your guidance and ideas........

Hello! I am just researching the AOS process myself, though we are doing the K1 visa route so don’t know if the AOS process is different for you doing the K3. I will just give my understanding about the medical requirements.

The medical you did in India is only valid for 12 months, so you will need to have another medical done here in USA. Make an appointment with a civil surgeon authorized by the USCIS; lists of doctors available here:

Costs vary widely, so call a number of doctors in your area and get estimates of costs if possible. Tell them you are applying for AOS and they will know what to do. They will let you know what vaccinations you need and they will have the I-693A they will fill out. About the DS-3025, you do not need to worry about it, because anyway you need a new medical again. Just let them know your situation and if you had an vaccinations before.

You must have filed before the year is up as I understand it.... So depending on when you had the medical done. You could contact the doctor that did you medical before have them send it to you. You can also go to a doctor and get titres done... could be costly though. Often times the CS will have copies of the forms they need. You will need to have the Supplemental form to I-693 for your interview if you have one. So the form is pretty much the same as the DS one except for the layout being portrait instead of landscape. But here it is http://www.cdc.gov/ncidod/dq/pdf/ti-03/appdx-a_693vacc.pdf . I know that the actual I-485 form instructions suggest that you MAY include the vaccination form. There is the potential for an RFE though when you don't include any vaccinations. I hope my scattered brain helped you a little.

Thanks to the both of you for your suggestion..

But i recollected that i did my medical on 14th July 2006, so 12 months for me is still not over..

I called up my parents back home and came to know that he has this sheet " Vaccination Summary" with him, i guess somehow i forgot to add that paper to my files..

I guess thats only the DS 3025 which will help me get my I693 A done from the local doctors..

Thank God..been pretty tensed abt not having the form with me.

Getting a scanned copy soon.

and for ELW - I am here on K1 as well...

That's good.... you can send in that DS form to the CS in Austin... who will complete your form and mail back to you for 10 dollars. That by far is the cheapest I've seen.

I called today but all doctors offices are closed today..

So I will be calling up tomorrow and asking them about the cost for preparing the I 693 A supplement..

If they are expensive then i would send my Vaccination summary to the Doctor in Austin, he does it only for $10.

So, I will let you know the Doctor's name and contact if there is good one, after i call them tommorow..
