I am completely p.....d off!!! I am trying to reach a civil surgeon to have my DS-3025 transferred to a I-693A.

I called that one place in my area. Nobody speaks english, they dont understand what I mean, finally i get a reach of someone that understands and she said its 300$ for the medical. I explain to her my visa (K-1) doesnt require a new medical to be done and that I already have the sheet. She goes to get informations and she comes back saying they dont do that. Only a full medical. I explain to her thats in the USCIS instructions, that I got her phone number from the USCIS website, but she says they cant. So I asked her if USCIS told me to call her to do that what should I do now if they dont want to and she says she doesnt know. I said nevermind I will call elsewhere. :angry:

The other place I reached wont tell me on the phone i have to get an appointment to show them what I want so that they can price it.

Anyone knows a nice place around Chicago suburbs where they didnt have trouble ?

I ran into same problem with phone people, I just took a copy of the instructions for form I-485, highlighted the part in the instructions about fiancee K-1 visas. When I pointed to the instructions the doctor stopped insisting on a medical and did what the instruction said and transfered the vaccination records to the supplement, sealed it in an envelope, and charged me $35 to do it.

Don't let them railroad you into a full medical.

I just called another palce and the girl was really nice on the phone but she said 60$. I might send my stuff to that guy in Texas for 10$ , still cheaper to pay for the shipping fees !

Be wary of that, when I visited the civil surgeon he insisted that my wife be there with passport to confirm that she was the person that was adjusting status.

Make sure they are a civil surgeon approved by USCIS.

They don't speak English in the suburbs of Chicago?

Have you considered coming downtown? There are several CSs in the city that might be able to help you. I haven't done it yet so unfortunately I don't have a name to pass on, but it might be worth the trouble.

Can you give me an idea how the DS-3025 looks?

Does it have your name and details and the physicians signature along with other details there?

I was never given a DS-3025 at the hospital where i got my medicals done in india i guess..all i have is a document with the heading -

"REQUIREMENTS FOR ROUTINE VACCINATION OF IMMIGRANTS EXAMINED OVERSEAS WHO ARE NOT FULLY VACCINATED OR HAVE NO DOCUMENTATION (all vaccines may be given at the same time, but at different sites on the body)"

Then the lists of vaccines for various age groups mentioned and the some of them encircled (guess those are the one applicable and i already have)

Below the doctors name is written but without his signature.

No where on this document is DS-3025 mentioned...

is this the right doc?

Please let me know how the DS-3025 looks so that i get an idea whether i am holding the right document to seek the vaccination supplement I 693A.

Unfortunately my husband would have to take time off of work for that and he doesnt have much time left and I cant use is car yet. Im going to one place tomorrow morning i have an appointement at 11am they will tell me how much it costs. If not i found another place that will do it for 60$ but I might consider the guy in texas for 10$. And yes I verified he is on the list of USCIS for civil surgeon.
