So, I'm finally going to file the report of marriage. I know I have to get a notarized form since we waited over a year, but I can't find the forms. I'm in Montana, so it should be the San Francisco embassy the website half the links don't work! 

 

Also, I know certain forms have to be notarized? Do I notarize copies or just originals? Thanks everyone!

I will let others comment on the forms but a notary attests to the live signature on a document not copies of a notarized document

The above form needs to be notarized and you can find the place for the notary to sign and affix their seal at the bottom of the first page. Embassy will want an original document.

Hello.. I am a filipina.. going to do rom too here in Minnesota this coming consulate outreach on the 17th. I tried to search for your case and found this..

We put N/A.  That is for individuals who acquired Philippine citizenship through naturalization.  You haven't, and she received Philippine citizenship through birth, so it is N/A.

 

Single is correct.
