As per their website:

 

The schedule of fees is as follows:

Each applicant 15 years of age and older: Bs. 150,000.00

Each applicant under 15 years of age: Bs. 60,000.00

This amount includes X-rays, blood tests and the medical examination itself.  If the examining physician considers that other tests are necessary, you must pay for these separately.  Children under 15 years of age do not need to have X-rays taken nor blood tests conducted, unless the doctor considers them necessary.

 

If you are applying for a K or V visa, vaccines are optional.  However, it is often more convenient if you are vaccinated while still in Venezuela (or Aruba/Curacao).

The authorized panel physicians selected by the Consular Section are the only doctors authorized to conduct medical examinations for visa applicants.  These doctors must verify that each immigrant visa applicant has met all vaccination requirements.

The cost of the vaccinations varies according to the age of the applicant.  The applicant is responsible for paying the appropriate fee upon arrival at the authorized panel physician’s office based on the doctor’s determination if you need any, all, or none of the normal vaccinations.
