Regarding Part 4, Information about Beneficiary, "Other Address and Contact Information" - lines 13a-13f:  My wife is Russian (with Russian Passport) but has lived in Thailand for 12 years (the last 6 years at the same and current address - as entered in Beneficiary's Physical Address (11a-11f). Should I enter her Permanant Russian Address back home in Russia in lines 13a-13f or just leave blank because she hasn't lived there for many years?  

 

Regarding Part 4, Information about Beneficiary, 57a-57f ("...... native written language ....), should we fill out this section entirely in Russian only, including the Thailand address translated into Russian? Or write the Thailand address as it is written in Thai, but the name portion (57a-57c) in Russian.

 

in the Information about You section (and also for my parents), it asks for City/town/village of birth (line 7 for example) - should I write Chicago only or Chicago, IL ? Same for parents City/town/village or residence (line 28)?

 

lastly,  in Place of your Current Marriage, we were married in Bangkok, Thailand (lines 19a and 19d). under Province (19c) should we write N/A since Bangkok is technically a Special Administrative Area and not a province, although it this designation is basically the same as a province? https://en.wikipedia.org/wiki/Provinces_of_Thailand.

If she hasn't lived in Russia for more than five years, you will not provide any Russian address.

 

Native alphabet, enter her name in Cyrillic and her address in Thai.

 

Yes, enter Chicago IL etc.

 

I would enter Bangkok as the province.

Either of those or leave blank.  All are good.
