there is no charge for the form. you are being charged for the civil surgeon entering your vaccination records on the form. you can try calling other civil surgeons to see if they charge a lower fee.

there is no charge for the form. you are being charged for the civil surgeon entering your vaccination records on the form. you can try calling other civil surgeons to see if they charge a lower fee.

We used our own service provider because the charge for the TD shot is covered under our insurance. There is no fee for the I-693A form, but there is a nominal charge/fee for tranfering the data (we paid $13). Please check out my wife's profile for the Civil Surgeon Locator to check out more USCIS approved civil surgeons in your area. If you decide to use a private doctor for to administer the TD shot then have the doctor write a note indicating that the TD shot has been administered and make sure the doctor signs and dates the note. Take that note along with your DS-3025 to the civil surgeon's office in order to have them transfer your vaccination history to the I-693A.
