I don't have an uscis account number and neither do I have other name? can I leave it blank.. Form doesnt let me put N/A too..  Can anyone show me a i751 sample form.. ?? I am very nervous..

Yes, leave it blank. It only doesn't let you type in the pdf. You can always print it out and write with ink.

Some PDF doesn't let you use special characters such as /. I suggest filling in the application before downloading it as pdf or before printing it.
