Form DS-11 Questions

I am about to appy for my passport, but i do have few questions about the form :

1. #9 >>> List all other names you have used : So i have to write down my previous maiden name before i got married?

And then on the STOP! Continue to page 2 Do not Sign xxxxx, and then there is Driver License, State Issued ID Card, etc >> Should i check them or just leave it blank until the authorized agent ask me to do so?

 

on the next page : Name of Applicant >>> is this my name again?

 

#19 : Permanent Address >>> My address is same with my Mailing address, so should i still need to write this #19  or just leave it blank, because it stated if residence is different from mailing address

Yes.  "Maiden name" is included in the instructions for that part.

Do not sign it until the agent asks you to.  Do not check anything off.  This is their part to fill out when you present your application to them. 

It states so very explicitly in the instructions for that part.

As stated in the instructions for this part, you fill in this section ONLY IF your mailing address is different than your residence address.
