I see some who think they can file their marriage "while visiting the Philippines" ...  rude awakening if they try that one.

Where did you see 3 months to file ROM or an affidavit is required.

Philippine Embassy, proper,  states   more than 1 year for ROM

http://www.philippineembassy-usa.org/philippines-dc/consular-services-dc/faq-dc/#marriage

And the consulate in New York states 1 year also

http://www.newyorkpcg.org/the-consulate/our-services/civil-registry/report-of-marriage

5.Notarized Affidavit of Delayed of Registration, if application is filed more than 1 year after marriage

We sent ours in 2016. The form I downloaded from their website was different back then. The report of marriage and report of birth was on the same form and the applicant crosses out what is not applicable. I also messaged them on facebook if I'm filling out the form correctly and they said yes. They have obviously updated the forms and made them separate together with the requirements (not 3 months but a year), still it stands to best check the state with the jurisdiction and their website.

Be interested in seeing that form.     There were individual forms in 2011 and onward that I ever saw.   New forms are slightly different but still ROM and ROB are too different to be a combo form

Sure.

And if you look at the website of the Philippine Consulate General in Chicago, the delayed report of marriage/birth/death form is also on the same page (not separate). I made a simple google search back in 2016 and the PCG in Chicago website was mentioned in a filipina's blog. :)

That isn't a ROB or ROM form.   That is an affidavit

"The report of marriage and report of birth was on the same form and the applicant crosses out what is not applicable"

Oh, "report of (x)", in my mind is the same as registration. In some consular websites they refer it to download forms for delayed registration, they don't stick to the word affidavit.

Well, if they go to the consular website like my original point, it says there all that needs to be done.

That website shows two separate forms also for ROM and ROB  .... not a one form serves all

You seem to be confusing the affidavit with the actual ROM & ROB forms

You're the one that jumped in here  to post :

"We sent ours in 2016. The form I downloaded from their website was different back then. The report of marriage and report of birth was on the same form and the applicant crosses out what is not applicable. "

I simply replied to ask to see a copy of this combo ROM & ROB form you used.  You produced an Affidavit for late registration document ... which is not a ROM or ROB form .. .or even a combo form.     My point was and still is, ROM & ROB are separate forms and you are confused as you keep referring to the Affidavit.     I will leave this at you are very confused.  ;)

3 things:

1) you asked about how mine said a requirement of 3 months, you said where did you see that, the website says 12 months, etc?!

2) this i believe threw me off course and mistook the affidavit as rom since it is a requirement of rom. I described the affidavit while using the word rom.

3) whatever inaccuracies I may have stated, one can fix those simply by following what the consulate geneal website states.
